All payments and co-pays are due at the time of the appointment. We accept all major credit cards and checks. Our office also sends out statements on a monthly basis.
Requesting a Copy of Medical Records
From time to time, everyone needs a copy of medical records. Whether your child is enrolling in school or going to see a specialist, we can issue a copy of their medical records. According to HIPAA regulations, we will require a medical release form before issuing any records. These forms can be found in Parents Magazine and through the Patient-Centered Medical Home that we are associated with. These resources also provide vaccine education for parents and guardians.
